Please provide us with your most recent relevant medical records including diagnostic imaging (e.g., MRI, CT), blood tests and prior consultations.  

Your information can be faxed, emailed or mailed to our office as shown below. For out-of-region patients, your CT and MRI images can be uploaded electronically to a secure server for our physicians to view. If some tests have not been done, our Clinical Coordinators Maricela Sandoval Sandovalm@jwci.org or Aubrieanna Redick Redicka@jwci.org can help you arrange these as well.
